Case Description
Had RCT on #3 end of March by endodontist. Pressure/pain continued. Redo in early May and canals left open. No relief of symptoms, infection found two wks later. Tooth cleaned and closed; no cracks noted under microscope. During all this, 3 rounds of antibiotics and predisone, alot of ibuprofen, etc. Some relief, but symptoms persist. Endo recommends extraction. How long can I walk around like this without intervention (don't want to cause more damage)? Why not an apico? How will I know if I'm a candidate for implant? Oral surgeon or who else? FYI: sinus looks like it hangs low into upper gums, first RCT looked like overfill but I was told this would be absorbed into body w/o problems -- is this so? Help!

I received your question via Dr Oogle. It''''s always frustrating when you are trying to do the best for your oral health and things just don''''t seem to be going as planned. Every once in awhile, for a variety of reasons, endodontic treatment just isn''''t successful. Your question indicates that you have done your research. My advice would be to see the general dentist who is ultimately going to restore the space where #3 will be extracted. I wouldn''''t recommend and apico if the endodontist advised extraction, it might be a little like investing in a sinking ship. Assuming your periodontal health is good and #2 and #4 are present and in good health, you probably have two options, a fixed bridge or an implant (assuming you are a non smoker). Your general dentist will be able to x ray the area and give you an idea of your options. If I believe that a patient is a good candidate for an implant I will then refer to a periodontist (who would actually place the implant) for a quick eval of the site. Usually the tooth can be extracted either at the general dental office or at the periodontal office (assuming they are placing the implant). I wouldn''''t worry about the sinus until you''''ve been evaluated by someone who deals with implants regularly. I''''ve run out of space to reply on Dr Oogle.
