In answer to your question, yes. A four year old has all of his or her baby teeth and these teeth should be repaired in order to hold important space in the arch until the permanent teeth replace them; starting at around six years of age. The last of the permanent teeth, other than wisdom teeth, will have erupted by about twelve years of age. Without the conservation of space in the jaw, the permanent teeth will not have the necessary room to erupt in their appropriate positions. This can cause significant problems later in life.

An important consideration for you should be: Why does my four year old have cavities and what can I do to prevent them? These days we no longer consider cavities to be mysterious and unavoidable. Sugar is the culprit, and a toothbrush is the solution. It is hard to avoid refined sugar, but if you and your child can keep the plaque from accumulating around the necks of his or her teeth at the gumline, your child will not have any more cavities.

Most importantly, before rejecting the advice of your dentist, you should consider the health and comfort of your child. I consider it to be child abuse to knowingly allow a simple cavity to progress to a painful infection. -Darrell Pruitt DDS
